The Rapid Emergency & Acute Care therapy team are looking for a Band 4 Therapy Support worker to join our team. We are a therapy service who work in accident & emergency, the adult assessment unit and some outpatient clinics with a keen focus on admission avoidance, frailty, & providing exceptional patient assessment & care. The clinical areas covered by the REACT Team are fast paced and the successful candidates should therefore be forward thinking and pro-active with excellent organisational skills. You will work autonomously as well as with Occupational Therapists and Physiotherapists in undertaking assessment, to facilitate admission avoidance and safe discharges and providing rehabilitation in the acute stages through to discharge planning and implementation. Excellent communication skills and flexible working are also required. Our therapy support workers are expected to: - complete face to face interventions with patients to gain background information - liaise with family, carers or community services - support qualified staff to assess mobility, transfers and functional tasks - equipment provision to support discharge - provide input to maintain function, or rehabilitate patients during their inpatient stay - complete referrals to other services - liaise with the multi-disciplinary team The successful applicant will be required to work 37.5 hours a week within a 7 day and extended hour service.
